You may enter this Sweepstakes via the Internet by logging on to www.sarasotarx.com, following all entry directions, providing all required information (including, without limitation, name and email address and where you can be contacted on or after the Drawing Date, as defined below), and submitting your entry. Incomplete, illegible, false/deceptive, and ineligible entries are void and will be disqualified, as are any entries made (a) by any method other than that set forth in this Rule Number 1, and/or (b) via any automated sweepstakes entry service or program(s).

ONLY TWO ENTRIES PER PERSON/ADDRESS WILL BE ACCEPTED. a) One Entry for answering the 5 survey questions and b) one additional entry for LIKING Sarasota Specialty Pharmacy and Wellness Center on Facebook.  Both a) and b) above need to be fulfilled for two entries to be registered, if only b) is fulfilled no entry will be entered. Sponsor's computer shall serve as the official time keeping device for the Sweepstakes. All entries submitted become the sole property of the Sponsor, and will neither be acknowledged nor returned.

ELIGIBILITY: To enter the Sweepstakes, you must be a U.S. citizen or lawful permanent U.S. resident residing in the 48 contiguous United States or the District of Columbia throughout the entire Sweepstakes Period. You must be an employee of a practitioner (physician, dentist, podiatrist, or veterinarian) to qualify. The practitioner must be in Sarasota, Manatee, DeSoto, Pinellas, Hillsborough, or Charlotte counties in the State of Florida. You must also be 18 years of age or older as of the commencement of the Sweepstakes Period. Sweepstakes is void in Puerto Rico, the U.S. Virgin Islands, U.S. Military installations in foreign countries, all other U.S. territories and possessions, and where restricted or prohibited by law. This Sweepstakes, and any website pages and advertisements relating thereto, is intended for viewing only within the continental United States. United States law governs this Sweepstakes. Employees of Sarasota Specialty Pharmacy, and their Immediate Families (as defined herein) and household members (whether or not related to such employees), are ineligible to participate in this Sweepstakes. As used herein, “Immediate Families”; means spouses, mothers, fathers, siblings, children, step-children, uncles, aunts, nephews, nieces, grandparents and in-laws, regardless of their place of residence

DRAWING AND NOTIFICATION: The potential Prize winner will be selected from among all eligible entries received in a random drawing to be held on or about the 1st of each month, until contest discontinued, by Sweepstakes Administrator whose decisions are final and binding in all respects. Odds of winning depend on the number of eligible entries received. The potential Prize winners will be notified by telephone and/or email. Failure by the potential Grand Prize winner to respond to such notification within a reasonable time frame, as determined by Sponsor in Sponsor's absolute and sole discretion, will result in disqualification of the potential Prize winner and alternate potential winner will be selected. The return of a potential winner's notification, prize information, or the prize as undeliverable, or the rejection of the prize by the potential winner will result in disqualification of the potential winner and the selection of an alternate potential winner. Sponsor reserves the right to modify the notification procedures in connection with the selection of any alternate potential winner. In the event of a dispute about the identity of an entrant, each entry shall be deemed to have been made by the Authorized Account Holder of the Address on the time and date of entry. The “Authorized Account “; is defined as the natural person to whom the Address was assigned by an Internet Service Provider, online access provider, or other organization (e.g., business, educational institution, etc.) that is responsible for assigning email addresses for the domain associated with the submitted Address. Sponsor reserves the right to require that the winner provide proof that (s)he is, and at the date and time of entry was, the Authorized Account Holder of the Address associated with the winning entry

PRIZES: One (1) Prize will be awarded consisting of the following: Kindle Fire.  Award must be awarded in the office of the practitioner.  Picture of awarding ceremony and name may be published on our web site.

SWEEPSTAKES CONDITIONS: VOID WHERE PROHIBITED. This Sweepstakes is subject to all applicable federal, state, and local laws of the United States. By participating, entrants agree to be fully and unconditionally bound by these Official Rules and the decisions of the Sponsor, and waive any right to claim any ambiguity in these Official Rules or the administration of the Sweepstakes. Entrants also hereby release, discharge, indemnify and hold harmless the Sweepstakes Entities and each of their respective officers, directors, employees, representatives, attorneys and agents from and against any and all claims, damages and losses of any kind including, without limitation, attorneys' fees, and costs of litigation and settlement, as well as any liability due to any injury(s) and/or disability(s) to any person (including death) resulting in whole or in part, directly or indirectly, from (i) the participation in this Sweepstakes and/or in any Sweepstakes-related activity, (ii) printing, distribution or production errors in these Official Rules or in any Sweepstakes materials or documents; and (iii) the redemption, acceptance, possession, ownership, or use/misuse of the Grand Prize or any portion thereof. Sponsor and/or the Sweepstakes Entities, in their sole discretion, may rescind all or any portion of this promotion found to contain such errors without liability of any kind or nature.

NEITHER SPONSOR NOR THE SWEEPSTAKES ENTITIES MAKE ANY WARRANTY, REPRESENTATION OR GUARANTEE, EXPRESS OR IMPLIED, IN FACT OR IN LAW, PERTAINING TO THE USE OF THE GRAND PRIZES OR ANY PORTION THEREOF, INCLUDING, WITHOUT LIMITATION, THOSE PERTAINING TO QUALITY, MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE. FURTHER, SPONSOR AND THE SWEEPSTAKES ENTITIES EXPRESSLY DISCLAIM ANY AND ALL RESPONSIBILITY FOR ACTS, OMISSIONS, OR DELAYS OF ANY GOVERNMENTAL OR QUASI GOVERNMENTAL AUTHORITY, INCLUDING, WITHOUT LIMITATION, ANY COSTS OR EXPENSES INCURRED AS A RESULT THEREOF OR IN CONNECTION THEREWITH.

RESTRICTIONS: At Sponsor's sole and absolute discretion, Sponsor may provide different prizes of equal or greater value. Sponsor shall not be liable to any winner or any person claiming through such winner for failure to supply the prize or any part thereof by reason of any acts of God, any action(s), regulation(s), order(s) or request(s) by any governmental or quasi-governmental entity (whether or not the action(s), regulations(s), order(s) or request(s) prove(s) to be invalid), equipment failure, threatened or actual act(s) of terrorism, air raid, blackout, act of public enemy, earthquake, war (declared or undeclared), fire, flood, epidemic, explosion, unusually severe weather, hurricane, embargo, labor dispute or strike (whether legal or illegal), labor or material shortage, transportation interruption of any kind, work slow-down, civil disturbance, insurrection, riot, or any other cause beyond Sponsor's sole control. 

LIMITATIONS OF LIABILITY: Neither Sponsor nor any of the Sweepstakes Entities are responsible for: (1) lost, late, misdirected, undeliverable or incomplete entries due to system errors or failures, faulty transmissions or other Internet or telecommunications malfunctions; (2) technical failures of any kind; (3) failures of any of the equipment or programming associated with or utilized in the Sweepstakes; (4) unauthorized human and/or mechanical intervention in any part of the entry process or the Sweepstakes; (5) technical or human error which may occur in the administration of the Sweepstakes or the processing of entries; or (6) any injury or damage to persons or property which may be caused, directly or indirectly, in whole or in part, from entrant's participation in the Sweepstakes or receipt or use of any prize. If for any reason an entry is confirmed by the Sweepstakes Administrator to have been erroneously deleted, lost or otherwise destroyed or corrupted, entrant's sole remedy is to submit another entry during the Sweepstakes Entry Period. Sponsor may prohibit an entrant from participating in the Sweepstakes or winning a prize if, in Sponsor's sole discretion, it determines that said entrant is attempting to undermine the legitimate operation of the Sweepstakes by cheating, hacking, deceitful, or other unfair playing practices (including the use of automated quick entry programs or services), or if the entrant is or intends to annoy, abuse, threaten or harass any other entrant(s) or any representative(s) of Sponsor/any of the other Sweepstakes Entities. If for any reason this Sweepstakes is not capable of being run or implemented as planned, including, but not limited to, as the result of infection by computer viruses or bugs, tampering, unauthorized intervention, fraud, or any other cause beyond the reasonable control of Sponsor, which, in Sponsor's judgment, is likely to or actually corrupts or affects the administration, security, fairness, integrity or proper conduct of the Sweepstakes, then Sponsor reserves the right to cancel, terminate, modify or suspend all or any part of the Sweepstakes, and to select a potential winner from among all eligible entries received by Sponsor up until the time of such cancellation, termination, modification or suspension, as applicable. ANY ATTEMPT BY AN ENTRANT/PERSON TO DELIBERATELY DAMAGE THE WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES MAY BE IN VIOLATION OF CRIMINAL AND CIVIL LAWS. SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO PURSUE ALL AVAILABLE RIGHTS AND REMEDIES, WHETHER CIVIL AND/OR CRIMINAL, INCLUDING DAMAGES, ATTORNEYS' FEES, AND COURT COSTS, AGAINST ANY SUCH ENTRANT/PERSON.

GOVERNING LAW: The Sweepstakes and these Official Rules will be governed by the laws of the State of Florida without regard for its conflict of laws rules. In the event that any dispute arises regarding the Sweepstakes or the Prize, the federal and/or state court of appropriate jurisdiction located closest to Sarasota, Florida shall have exclusive jurisdiction with respect thereto. Any dispute(s) of whatever kind or nature in connection with this Sweepstakes shall be brought individually without resort to any form of class action.

PRIVACY POLICY AND TERMS OF USE: BY ENTERING THIS SWEEPSTAKES, EACH ENTRANT ACKNOWLEDGES AND AGREES THAT: (i)

THE ENTRANT HAS OPTED-IN TO SARASOTA SPECIALTY PHARMACY'S PRIVACY POLICY ON THE SWEEPSTAKES ENTRY PAGE; AND (ii) BY OPTING-IN, THE ENTRANT HAS READ AND CONSENTS TO SPONSOR'S PRIVACY POLICY AND TERMS OF USE. EACH ENTRANT FURTHER ACKNOWLEDGES THAT IF HE/SHE IS CHOSEN AS A WINNER, CERTAIN OF HIS/HER IDENTIFYING INFORMATION MAY BE DISCLOSED TO THIRD PARTIES AT LEAST AS REQUIRED BY LAW INCLUDING, WITHOUT LIMITATION, ON A WINNER'S LIST.
